To help us achieve this, we need to find out as much as possible about your health so we can provide the most appropriate treatment. This includes your medical history and surgical photographs during your consultation and procedure.

4.2 When requesting a consultation over the phone
When you call our office to make an appointment, we may ask you for Personal Information such as your name, email and telephone number. Personal Information obtained in this way is held online in our password-protected system. We do not keep hard copies of your information.
4.3 During your consultation
Your medical history will be obtained during your consultation. This information is stored in our online secure system and is combined with information from any previous consultations that you may have had with us along with any test results and referral letters.
5. What information is not secure?
We endeavour to protect the privacy of your account and other Personal Information that we hold in our records, but unfortunately, we cannot guarantee complete security. Unauthorised entry or use, hardware or software failure, and other factors, may compromise the security of user information. Also, while we endeavour to put adequate contractual protections in place we cannot guarantee the security of any Personal Information in databases hosted by third parties.

8. How long is my Personal Information retained?
We retain your Personal Information for as long as we provide services and/or treatment to you and in line with legal and regulatory requirements or guidance.
For most patients, information is retained for a minimum of eight years from when you were last seen. Some types of information are retained for a longer period. For example, children’s records must be retained for at least 25 years and cancer records for 30 years.
